  • Complete all sections of this application accurately. A fully completed application is required for consideration.

  • All correspondence and award agreements will be prepared using the information you provide. Please use proper spelling, capitalization, and punctuation.

  • After completing the application, save it as a PDF and email it to osf.scholarshipforms@gmail.com along with all of the following required documents:

    1. This application, fully completed and submitted as a PDF.

    2. Letter of Admission to your institution.

    3. Transcript or academic results from your previous year or level showing your courses and GPA of 2.5 or greater.

    4. A letter from your institution or a Deaf and Hard of Hearing student association confirming your membership and that you are Deaf or Hard of Hearing. For Konyinsola Ajayi, SAN Fellowship, proof of a documented disability endorsed by your institution.

    5. An original two-page personal statement or a 3–5 minute sign language video describing your life goals and aspirations. Non-original or plagiarized submissions will result in disqualification.

  • All required items must be received for your application to be considered. Any incomplete application, missing documents, or failure to meet these requirements will be deemed incomplete and will not be reviewed for consideration.

  • *OSF reserves the right to verify all information and supporting documents with the issuing institution or organization. Submission of forged, altered, falsified, or otherwise fraudulent documents—including transcripts, admission letters, recommendation letters, or disability verification—will result in immediate disqualification and permanent ineligibility for all current and future OSF Scholarship Programs.
